| Who may attend: |
The target audience for this course are ATSDR staff and ATSDR state partner health assessors who will be conducting, or have conducted, public health assessments, health consultations, and exposure investigations (i.e., exposure assessments and health effects evaluations). Public health professionals who prepare, review, or edit public health products or who conduct other health assessment/health study/community involvement/health education promotion or other intervention activities as part of the public health assessment process or general environmental public health are also welcome to attend. |

| Field Dress: |
As part of a field practicum, we will be making a site visit on Wednesday, June 10 (RAIN OR SHINE). We have arranged a bus for travel. The site we are visiting IS NOT a real hazardous waste site or a real petitioned site. However, for the purposes of the field practicum, we will assume that ATSDR has been petitioned by the community to prepare a public health assessment on the site. You WILL NOT need hard hats, steel toed shoes, or other personal protection equipment, and you will be allowed to enter the site area without having to have completed any prior OSHA training. All participants should bring a light rain parka in the event of rain. We also recommend that you bring a hat/cap and sunscreen as Atlanta is usually hot and sunny in June. We will furnish coolers of water so canteens will not be necessary. Be prepared to be at the site at least four hours. We will be walking over the site and doing some field exercises. Field boots, sturdy tennis shoes or other comfortable walking shoes will be needed for hiking and crossing the streams. Older blue jeans or other heavy duty pants should be worn. The site has poison ivy, pollen-producing weeds/trees, briars, snakes, domestic farm animals, mosquitoes, chiggers, and ticks. If the weather is rainy, you may also have some mud to walk through. (The site is not really as bad as it sounds; we just want you to be prepared.) |

| Accommodations: |
We have reserved a block of rooms at Atlanta Marriott Century Center, our primary hotel. The rooms must be reserved by Monday, May 12, and you should ask for the "CDC/NCEH ATSDR Basic Course" rate to receive the discounted rate of $124 (single or double occupancy). Note that the negotiated rate is less than the government per diem rate of $141. Cancellations must be made no later than 24 hours prior to the arrival date. Reservations must be made by individual attendees directly with Marriott reservations at 1 (800) 228-9290 or online. You may need to bring a tax exemption form to avoid paying additional state taxes. Discuss what documentation the hotel will accept from your state for tax exempt purposes when you make the reservation. Also, discuss what forms of payment the hotel will accept. Requests for specific room types will be honored on a first come, first served basis (e.g. non-smoking, etc.). | Transportation: |
The AAA Airport Express (404-767-2000 or 800-354-7874) can be used to travel between the hotel and airport. The cost is at least $35 one-way. Rates and schedules are available online at AAA Airport Express. You should call ahead of time to schedule transportation. If you do not wish to use this service, a taxi or rental car will be needed for transportation. You may want to check with your hotel too to see if they offer gratis shuttle service. Transportation to Century Center is also available through the Metropolitan Atlanta Rapid Transit Authority (MARTA) system ($1.75 each way). Click here to view MARTA itineraries and maps. |

Everyone must complete the online registration form to register. Please make your plans quickly, as space is limited and demand may exceed available space. REGISTRATION DEADLINE IS MONDAY, June 1, 2009. Training manuals and name tags will be distributed the first morning - Monday, June 8, 2009, from 7:30 AM until the course begins at 8:00 AM. |

| Continuing Education Certification |
This course is accredited for the following: CHES: CDC is a designated provider of continuing education contact hours (CECH) in health education by the National Commission for Health Education Credentialing, Inc. This program is a designated event for the CHES to receive 34 Category I Contact Hour(s) in health education. CDC provider number GA0082. CEU: The CDC has been approved as an Authorized Provider by the International Association for Continuing Education and Training (IACET), 1760 Old Meadow Road, Suite 500, McLean, VA 22102. The CDC is authorized by IACET to offer 4 CEU's for this program. In order to get continuing education credits and/or a course completion certificate with the number of contact hours you must fill out an online course evaluation form. Note that you must be present for the entire course time designated in the agenda to receive a course completion certificate.
